CISA/CISM試験受験時の本人確認証(ID)に関するポリシー



ISACA東京支部は、2007年7月20日、国際本部から次のポリシーを受領しました。
CISA/CISM試験の受験者各位は、このポリシーに準拠してください。
但し、ISACA東京支部は、この日本語訳について、原文との同一性を保証するものではありません。原文と日本語訳の文意が異なる場合には、原文が優先します。

ISACA主催試験時の本人確認証（ID）ポリシー
CISA/CISM試験の全ての受験者は、有効な受験票と適切な本人確認証（ID）を持参し提示した場合にのみ、試験会場への入室が認められる。IDの形式要件としては、受験票に記載された氏名と、受験者の写真が含まれ、公的機関が発行し、現時点で有効、かつ原本であることが求められる。提示された一つのIDのみによって、これらの要件が全て示されるべきである。たとえば（但し次のものには限らないが）、パスポート、運転免許証、軍隊ID、州ID、グリーンカード、国家ID。適切かつ有効な本人確認証を提示しない受験者は、受験できず、また受験登録費用は返還されない。

ISACA EXAM IDENTIFICATION POLICY
Each candidate taking the CISA/CISM exam must submit and will only be admitted to the test center if they have a valid admission ticket and an acceptable form of identification (ID). An acceptable form of ID must be a current and original government issued identification that contains the candidate’s name as it appears on the admission ticket, and the candidate’s photograph. All of these characteristics must be demonstrated by this single piece of ID provided. Examples include, but are not limited to a passport, driver’s license, military ID, state ID, greencard and national ID. Any candidate who does not provide an acceptable form of identification will not be allowed to sit for the exam and will forfeit his/her registration fee.
